PVHC offers a full range of employment health screenings as part of our Employer Advantage occupational medicine program. Each screening is conducted by our experienced staff and overseen by a board-certified physician.

Employee Health Screening Patient Information

Here are some helpful reminders to ensure the best possible patient experience: 

• Appointments are required for all physicals (unless it is only a drug screen)
• Arrive on time or early to fill out all necessary paperwork
• Patients more than 15 minutes late for their appointments will be asked to reschedule
• No children allowed during the appointment
• Applicants under 18 years of age must be accompanied by a parent or legal guardian

What to bring:

• Referral authorization slip (plus additional paperwork as given)
• Valid Photo I.D.
• Glasses or corrective lenses (if worn for vision/reading)
