Employee Health Screening Patient Information
Here are some helpful reminders to ensure the best possible patient experience for your staff health screening:
- Appointments are required for all physicals (unless it is only a drug screen)
- Arrive on time or early to fill out all necessary paperwork
- Patients more than 15 minutes late for their appointments will be asked to reschedule
- No children allowed during the appointment
- Applicants under 18 years of age must be accompanied by a parent or legal guardian
What to bring to your employee health screening:
- Referral authorization slip (plus additional paperwork as given)
